Use the US dog/cat model from a USDA-accredited veterinarian with APHIS endorsement, both completed no more than ten days before arrival. A printed completed endorsed certificate accompanies the pet; digital endorsement is permitted but a phone copy alone is insufficient.

Implant and verify the microchip before or on the same date as the documented rabies vaccine; record number, location and implantation date. Use ISO-compatible identification or provide your own compatible reader if the accepted chip is non-ISO.

Maintain current rabies vaccination. Primary vaccination or a booster after expiry requires at least 30 days before departure, also satisfying the guide’s pre-import wait. A booster within previous validity does not require that wait.

A rabies antibody sample must be taken strictly more than 30 days after primary vaccination and yield at least 0.5 IU/mL at an OIE-approved laboratory. Keep the sample date and report. Exactly day 30 does not meet the model’s wording.

The linked US model permits antibody results to remain valid for the animal’s life while rabies vaccination stays current under manufacturer instructions. Keep the continuous booster history; a lapse cannot be treated as continuous coverage.

The certifying veterinarian verifies the microchip and examines the animal at certification, finding no infectious/contagious disease signs or known exposure. Complete the identity and vaccination tables; a blank downloaded model is not a certificate for the pet.

All US importers submit Annex B arrival notification and a copy of the APHIS-endorsed certificate to the entry-port veterinary office at least two working days before US departure. Obtain port authorisation for release; Annex B for Ben Gurion gives vs-airport@moag.gov.il.

US certificates cannot be issued for the listed dangerous dog breeds or their crosses without a valid translated Israeli permit specifically authorising dangerous dogs. The list includes Amstaff, Bull Terrier, Dogo Argentino, Japanese Tosa, Staffordshire Bull Terrier, Pit Bull, Fila Brasileiro and Rottweiler; specify mixed-breed type.
